Athletic Training

College of Saint Mary Sports Medicine renders the best possible medical care of our student-athletes in conjuction with the physicians at Children's Nebraska as well as the physicians and physical therapists at University of Nebraska Medical Center.

All student-athletes must have an up-to-date physical examination, completed physical packet and insurance card on file, in addition to being cleared by the head athletic trainer prior to the start of any CSM Athletic Department summer activities, conditioning, practices or competitions.

Please bring this physical form to your doctor for your exam: Physical Form Link

All students are required to download the app Healthy Roster. All pre-participation paperwork will be found here once you have an account. This is how we track injuries and how you will communicate with our athletic trainers. You will receive a link to your school email from Healthy Roster with an activation code. To make an appointment with an AT, please follow this link: https://dashboard.healthyroster.com/scheduling/csmatr/self-scheduler

You will be required to perform baseline testing via Sway Medical. This is our concussion software. You will do this on campus with the athletic trainer when you arrive on campus in the fall.
